Shortening of the Femoral Neck Following Peritrochanteric Fracture
نویسنده
چکیده
Proximal femoral neck and peritrochanteric fractures are a devastating musculoskeletal injury resulting in markedly increased patient morbidity and mortality. As the annual incidence of fracture continues to increase, there is a recognized need to improve clinical outcomes following fixation, especially in peritrochanteric fractures. While many factors are associated with adverse patient outcomes, the collapse and shortening of the femoral neck that can occur during osteosynthesis of neck fractures or in surgically treated unstable peritrochanteric fractures may represent a preventable complication. Femoral neck shortening is known to decrease the moment arm of the hip abductors reducing patient function. Contemporary internal fixation devices for neck fractures allow head-neck fragments in peritrochanteric fractures to slide along/together with the implant, increasing the risk of an unlimited compression and collapse. However, the newly developed INTERTANTM intertrochanteric antegrade nail improves rotational fracture stability and allows for intraoperative interfragmentary compression, thereby reducing the erosion of the fracture surfaces between the lateral end of the neck fragment and the trochanteric region that might result in uncontrolled collapse with the risk of shortening.

Incidence of and Factors Influencing Femoral Neck Shortening in Elderly Patients After Fracture Fixation with Multiple Cancellous Screws
BACKGROUND To study the incidence of and factors influencing "neck shortening" in elderly patients treated for femoral neck fractures using multiple cancellous screws. MATERIAL AND METHODS Of the 197 femoral neck fracture cases treated via closed reduction and cancellous screws fixation from January 2006 to February 2010, 110 were followed up. Patient age, gender, operative time, implantation m...
متن کاملDoes the PFNA™ nail limit impaction in unstable intertrochanteric femoral fracture? A 115 case-control series.
INTRODUCTION Femoral neck shortening after dynamic fixation of extra-capsular fracture may impair functional results, but is rarely assessed. The present study measured impaction in stable and unstable fractures (on the Ender classification) treated by PFNA™ nail. The objectives were: 1) to validate the Ender classification to assess fracture stability; 2) to determine whether neck shortening a...
متن کاملFemoral neck shortening after internal fixation of a femoral neck fracture.
This study assesses femoral neck shortening and its effect on gait pattern and muscle strength in patients with femoral neck fractures treated with internal fixation. Seventy-six patients from a multicenter randomized controlled trial participated. Patient characteristics and Short Form 12 and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores were collected. Femoral ...
متن کاملHip arthroplasty after treatment failure in intertrochanteric femoral fractures.
Background. Treatment failure in intertrochanteric fractures of the femur leads to pain and limitations of limb function. Methods of treatment allowing for union in order to preserve the proximal femur are undertaken in younger patients. Older patients who have poor quality bone stock and bone loss in the proximal femur are treated with hip arthroplasty. The aim of our study was to perform a lo...
متن کاملShortening After Femoral Neck Fracture Fixation
When using parallel screws for treatment of femoral neck fractures, shortening of the femoral neck might occur. Given the high revision surgery rates associated with parallel screws, we developed a questionnaire to explore (1) surgeons’ viewpoints on difficulties in the fixation of femoral neck fractures, (2) their perception of the clinical importance of femoral neck shortening after internal ...
